A R Caw obtained his MB BS from Adelaide University in 1902. He served during during World War I as a Staff Surgeon aboard HMAS Australia. He was promoted to Fleet Surgeon Commander in 1917. HMAS Australia became the flagship of the Australian Naval & Military Expeditionary Force which was separate from the Australian Imperial Force.
After the war ended, Caw resigned from his position in the Navy. He went to Johannesburg, South Africa and took up medical practice as a Specialist Ear, Nose & Throat Surgeon.
